How do I register?
If you have recently come to live in Aylesbury and you fall within our catchment area you can complete the relevant forms which are available on our website.
Are you taking regular medications?
If you take repeat regular medications prescribed by your previous GP, we would ask that you have at least 28 days worth of medications before registering with the practice. This will allow us time to arrange you a routine medications review before we issue your medications.

Important: Register Online
We are using a new online service called Register with a GP surgery that makes it easy to register with this GP surgery. Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number. The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

You can email this to admin.poplargrove@nhs.net or we are happy to accept these over the counter at Reception.
The forms are also available to collect in reception.
