How do I register?

If you have recently come to live in Aylesbury and you fall within our catchment area you can complete the relevant forms which are available on our website. Please check our catchment mapRequests to register that fall outside of our catchment will be rejected.

  • Online using the forms under Registration Forms 
  • Attending the practice and completing a registration pack

The form GMS1 is MANDATORY for all patients wishing to register and must be completed in full. If you have come from abroad, a date of entry to the UK must be provided.

We would prefer to receive your completed registration forms via email to admin.poplargrove@nhs.net (see note below) however we are happy to accept these over the counter at Reception. Alternatively you may wish to post them to us, or drop them in our post box located at the entrance opposite Rowlands Pharmacy.  

Please note; the above email address is used ONLY for new registrations, change of address/contact details and online access. Any other emails will NOT be picked up. 

If possible please scan and send a copy of your photo ID and proof of address along with the registration forms. 

Incomplete or illegible form will be returned to you unprocessed.

 

Alternatively you can register online.

We are using a new online service called Register with a GP surgery that makes it easy to register with this GP surgery. Just fill in this quick online form to start the process. You do not need proof of address or immigration status, ID or an NHS number.  The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.
