Can I register with you?                                                          

 

We are proud to be a 'Safe Surgery' for everyone in our community. All are welcome. 

You will need to reside within our practice boundary, use our postcode checker at the bottom of the page.

If you are living in one of the above areas find your local catchment GP surgery here.

If you have moved to the UK from abroad please click on this link for more information. Everyone in the UK has the right to free care from a GP. If you don't have proof of ID or address, you can still register. Immigration status or nationality doesn't matter. We do require your date of entry to the UK added to the GMS1 form to be able to process your registration.

Are you taking regular medications?
If you take repeat regular medications prescribed by your previous GP, we would ask that you have at least 28 days worth of medications before registering with the practice. This will allow us tiome to arrange you a routine medications review before we issue your medications. 

Staying less than 3 months?
If you are staying for less than 3 months and require acute medical assistance we would ask that you complete a GMS3 form which you can complete at reception or with a member of the Patient Services Team over the phone. You must be residing in our catchment area and with someone who is a patient at the practice already. You will be registered as a temporary patient which means we will not obtain your previous GP medical records. 

Have you moved into an address with residents reigstered at our practice?
From 1st August 2022, if you are over 18 years old and move into an address, where members of that household are currently registered at Poplar Grove Practice but the address is outside of our catchment, you will be asked to register at the surgery that officially covers your address.

New patients must live within the practice boundary to be eligible to register with us.

Existing patients who move to an address outside our boundary will be asked to register at a surgery that covers your address. Click here for more information.
