Moving Home
Effective from 1st August 2022, due to our increasing practice list size, we will no longer be able to keep patients registered at Poplar Grove Practice who move anywhere outside of our contracted catchment area. You can check if your new address is in our catchment area by entering your new postcode at the link here.
Once we are informed of your new address, if your new address falls outside of our contracted boundary, you will be sent a letter asking you to register at the surgery that covers your new address. You should register with your new GP within 28 days as after 28 days your name will be removed from our patient list.
