Crisis Home Assessment Team
formally Over 75's Team
The CHAT Team's role is assessing clients for practice colleagues for those clients unable to attend the surgery. We work in close partnership with Social Services as well as secondary care partners, social and private care agencies as well as families and carers. The role is extremely variable and always under review.
The team compromises of Samantha Cashman and Heather Thomas. You can contact the CHAT Team via our switchboard on 01296 468580 and those patients who are under the long-term care of the CHAT Team have access to a direct line with answer machine.
Samantha Cashman
Community Nurse
Sam's role is to access clients in their homes to establish what clinical or social support is needed in order to prioritise care and make referrals to ongoing services as needed. Sam has a specialist interest in Diabetes Mellitus and long-term conditions, her aim is to treat each client as an individual and assist them holistically involving the client, carers and family. Most referrals are taken from GP's however Social Care and Secondary Care referrals are becoming more common.
Heather Thomas
Community Healthcare Assistant
Heather works alongside Sam seeing people that can't get to the surgery. Heather attends patient homes to perform blood tests for management and monitoring of conditions including; Diabetes, Warfarin, DOAC's as well as performing ECGs and collecting samples.
