What do I do next?

Go to the section 'Registration Forms'; download and complete the forms, then email them to admin.poplargrove@nhs.net.

Please note; the above email address is used ONLY for new registrations, change of address/contact details and online access. Any other emails will NOT be picked up.

Or you can place them in our post box which is situated at the entrance opposite the pharmacy.

ALL forms are mandatory.

If possible please scan and send a copy of your photo ID and proof of address along with the registration forms.

Please note incomplete or illegible forms will be returned to you unprocessed.

If you taking any regular medications, on receipt of your confirmation email or letter, please contact the surgery on 01296 468 580 to arrange a telephone medication review. Please do not leave it too late to do this and risk being without your medications.

Are you a Carer for someone? 

At Poplar Grove we pride ourselves in not only looking after the patient but taking care of the carer that patient may have.  If you care for someone, whether they are registered at Poplar Grove Practice or not please complete our Carers Identification Form.  With your permission, we will pass your details on to Carers Bucks https://carersbucks.org/

Please note this does not apply to being a carer as an occupation.
