Registering process for Online Apps

A GP surgery can give someone proxy access so they can help another person manage their GP health and care.

To be able to register for these Apps you will need a linkage document from the practice which contains the information required to register.

• Linkage key
• ODS Code
• Account ID

To obtain this document we require a Patient Access from to be completed. The form is available here. Or by visiting the surgery, there is a form pick up rack in reception.
